Dog Release Agreement

This agreement is about taking your Camp Dog home with you throughout the Camp Session.

Please carefully read the Dog Release agreement below, and sign at the bottom of this form.

Dog Release Liability Form

  • I understand that Doglando cannot, and does not make any guarantees as to the health, temperament, mental disposition, and training of the dog. I hereby foster the dog at my own risk.

  • I fully and completely release Doglando, its founder and volunteers, from any and all claims and liability, including, but not limited to, any defects or illnesses the dog may have or develop, and for any injury or damage to persons, property, or other household pets, which may be caused by the dog. I further agree to indemnify and hold Doglando harmless against any and all claims and liability, including, but not limited to, those asserted by third parties for any injury or damage to persons or property caused by the dog.

  • I certify that all the animals in my home are current in their vaccinations and heartworm prevention. I fully and completely release Doglando from any and all claims.

Transfer of Ownership

  • I understand that I am acting as a temporary custodian of any and all dogs fostered for Doglando, and that Doglando remains the lawful owner of the dog. I will surrender the dog to the Doglando representative within 8 hours if requested to do so for any reason.

  • I will never abandon the dog. I will never give, sell, or transfer ownership of the dog to another person, relative, rescue group, humane association, shelter or pound, pet store, or any medical or experimental laboratory or similar organization.

  • If I transfer the dog without the knowledge and approval of Doglando, I understand that Doglando will reclaim the dog from the transferee and will pursue legal means to do so if necessary, and will pursue legal means to redress any harm done to the dog.

  • If I decide that I would like to adopt the dog, I will advise Doglando and follow all adoption procedures. I understand that while I have been permitted to foster the dog, this does not precipitate permission to adopt. I understand that Doglando makes the final decision regarding the adoption of this dog, which is based on the best interest of the dog.

Safety

  • I will make sure the dog is always wearing proper identification, which will be provided by Doglando, and the dog will always be on a leash. I will not take the dog to any dog facilities without the permission of Doglando. I will make sure the dog does not come in contact with any unfamiliar dog.

  • I will make sure the dog is crated when I leave my home and at bed time. No one other than myself will be taking care of the dog.

  • I will advise Doglando immediately if I plan to change my address and/or phone number. I will request University of Doglando's permission to transfer the dog to my new address.

  • I will advise Doglando immediately if the dog is lost or stolen, and make every reasonable effort to recover the dog, including, but not limited to posting flyers, and contacting local shelters and veterinarians.

I am aware that by signing this form, I agree to provide what is needed for the health and safety of the dog in my care for Doglando. I further agree to assume all responsibility for the dog in my care while on or off my premises.  I also agree to assume all liability for the dog while I foster it for Doglando.

I fully understand and agree to assume all risks involved in any and all duties that I perform for Doglando in my volunteer capacity and I agree to hold Doglando harmless for any injury(s) that I might sustain during the course of my volunteer duties.

This waiver does include myself, all of my family members and descendants forever from seeking any legal action whatsoever against Doglando or its Representatives.

I also understand that when I resign as a foster to return any equipment & unused supplies provided to me by Doglando.

By signing this application and waiver, I declare I am at least 18 years of age and agree to all terms listed list in the above contract.

Registration Payment Options

You can choose from a single payment (full pay), or make 3 monthly installments, which will take place today, and 30 and 60 days from today.
